about graduating medical high school. 2 Copy of documents about
postgraduate training on neurology (internship, doctoral studies, primary
specialization not less than 500 hours), given by an educational institution.* 3 Copy of the diploma of
the medical doctor or professor. 4 Copy of document about
graduating advanced training courses on neurology for last 5 years. 5 Copy of document
certifying medical category (the first or the higher). 6 Copy of the work-book. 7 The industrial
characteristic reflecting skills of the competitor, certified by the head
fisician. The phrase - ⌠for certification on a speciality ⌠neurology■ is
obligatory. 8 Copies before the given
certificates. *If you worked as the neurologist till 1995 on records
in a work-book copies documents about postgraduate training on neurology are
not obligatory. Documents to enclose in a separate folder. All copies of documents should
